DOT Exam Requirements for CDL Drivers

DOT exam hours: Mon-Fri: 9 am to 4 pm | Sat: 10am to 2pm

What Is the DOT Physical Exam?

The DOT physical exam ensures Commercial Driver’s License (CDL) holders are medically fit to operate commercial motor vehicles safely. Exams must be performed by a FMCSA-certified Medical Examiner

Who Must Take the DOT Exam?

-CDL holders operating commercial motor vehicles in interstate commerce
-New CDL applicants
-Drivers renewing or updating their Medical Examiner’s Certificate (DOT medical card)
-Drivers returning to duty after certain medical conditions or violations

How Often Is the Exam Required?

-Every 24 months (standard)
-More frequently (e.g., 3, 6, or 12 months) if the examiner requires monitoring for specific conditions (such as high blood pressure, diabetes, or sleep apnea)

What should you expect during your DOT exam?

-Valid photo ID
-Current DOT medical card (if renewing)
-Corrective lenses or hearing aids (if used)
-List of medications (prescription and over-the-counter)
-Medical records for chronic conditions (if applicable)
-Diabetes or vision waivers (if applicable)

Medical Requirements Checked

  • 20/40 acuity or better in each eye (with or without correction)
  • 70° peripheral vision in each eye
  • Ability to **distinguish red, green, and amber colors**
  • Ability to hear a forced whisper at 5 feet in one ear  
    OR
  • Audiometric testing showing acceptable hearing levels
  • Normal: Below 140/90 – up to 2-year certification
  • Stage 1 (140–159/90–99): Usually 1-year
  • Stage 2 or higher: Shorter certification or temporary disqualification until controlled

Screens for:

  • Protein
  • Blood
  • Glucose
  • Helps detect conditions like diabetes or kidney disease  

This is not a drug test.

Includes evaluation of:

  • Heart and lungs
  • Neurological function
  • Musculoskeletal system
  • Abdomen and general health
  • Ability to safely perform job duties (climbing, gripping, mobility)

Commonly evaluated conditions include:

  • Diabetes
  • Heart disease
  • Sleep apnea
  • Epilepsy or seizure history
  • Mental health conditions
  • Substance use or medication side effects

Some conditions may require **documentation, monitoring, or a federal exemption/waiver**.

  • Uncontrolled high blood pressure
  • Poorly managed diabetes
  • Severe vision or hearing loss
  • Recent heart attack or stroke
  • Untreated sleep apnea
  • Certain seizure disorders without exemption

Many conditions are not automatically disqualifyingif properly treated and documented.

  • Keep your DOT medical card current at all times
  • Report any significant health changes
  • Failure to maintain medical certification can result in:
    • CDL downgrade
    • Out-of-service orders
    • Fines or job loss
